• Simplified code fixers AM002, AM004, AM006, and AM011 from 5-10 lightbulb options down to max 2 per diagnostic.
• Each fixer now offers one contextually-chosen best fix plus one fallback (e.g. ignore/suppress).
• Removed bulk fix infrastructure (BulkFixConfigurationParser, grouped actions, nested code action menus).
• Uses WellKnownFixAllProviders.BatchFixer for multi-diagnostic scenarios instead of custom bulk logic.
• Fuzzy matching selects the single best candidate; falls back to default value or ignore when no good match exists.
• Deleted ~900 lines of code across fixers and base class while preserving all functional behavior.
